Background: Health care burden of Ventilator Associated Pneumonia (VAP) can be
represented by the prolonged hospital stay, long-term disability, and the massive additional financial
costs for the health care system. Aim: To assess the health care burdens of ventilator associated
pneumonia in Pediatric Intensive Care Units. Design: A descriptive design was utilized. Setting:
The study was conducted in Pediatric Intensive Care Units at Children’s Hospital that is affiliated to
Ain Shams University. Subjects: A purposive sample of children suffering from VAP (40 children),
a convenient sample of pediatric nurses working in the PICU (40 nurses), Tools: Structured
questionnaire sheet to assess nurses' demographic data, and their knowledge regarding VAP.
Assessment of children characteristics, treatment, and diagnostic studied; Observational checklist
for nurses' performance; PICUs environment physical assessment; and parameters of VAP health
care burdens. Results: The majority of the studied nurses had poor level of knowledge regarding
VAP, while about two thirds of them were incompetent regarding care of children with VAP. Most
of the studied children' age was less than one year, while the minority of them were more than 11
years old. The Incidence of VAP was 563 per 1000 children high risk for VAP. Total hospitalization
cost was 496696.8 LE for the total number of the studied children with VAP. Conclusion: Based on
the study findings, it could be concluded that, ventilator associated pneumonia had a great burden
among the mechanically ventilated children in the pediatric intensive care units, which is strongly
related to the poor levels of nurses' knowledge and practices regarding VAP. Recommendation:
Burden parameters of ventilator-associated pneumonia should be adopted as blueprints for PICU
staff adherence to the guidelines.

A clinical pathway is a multidisciplinary management tool based on evidence-based practice in
which the different tasks "interventions" by the professionals involved in the patient care are
defined, optimized. It can be utilized in education and facilitate access to care and providing
improved outcomes for patients and increasing women and staff satisfaction. Aim: The study
aimed to evaluate the effect of clinical pathway application on maternity nurses' performance of
intrapartum care and women's satisfaction. Design: A quasi-experimental design was used.
Settings: This study was applied in gynecology and obstetrics departments affiliated with Tanta
University Hospital. Sample: A purposive sample of 60 maternity nurses and 120 intrapartum
women were recruited in this study. Six tools were used for Data Collection: a structured
interviewing questionnaire for the nurses, a structured interviewing questionnaire for the
intrapartum women and clinical pathway checklist for maternity nurses, an observational
checklist for women, a Numeric Pain Intensity Scale, and maternal satisfaction with hospital
based intrapartum care scale. Results: Statistically significant differences were found between
pre and post-implementation of the clinical pathway reflected by higher knowledge and
performance score for nurses' improvement in the maternity nurses' knowledge regarding all
items of the postpartum clinical pathway after the intervention than before the intervention. The
majority of nurses adequately done all items of newborn care; assessment of newborn,
suctioning, Apgar score, cord care, eye care, measurements, body temperature, vitamin K
administration , and identification. Also, only (8%) of the intrapartum women suffered from
severe pain in a clinical pathway group compared to 38% routine care group. In addition,
intrapartum women's satisfaction in the study group has improved. Conclusion: using of
clinical pathway had a positive effect in improving the maternity nurses' performance and
women satisfaction of intrapartum care post clinical pathway application than pre-intervention.
Recommendations: Training programs should be implemented for maternity nurses about
intrapartum care based on the clinical pathway to improve nurses' performance.

Multiple sclerosis is a chronic disease affecting the central nervous system. Multiple sclerosis
occurs when the immune system attacks nerve fibers and myelin sheathing in the brain and spinal
cord. Aim: the current study aimed to evaluate the effect of nursing guidelines regarding safety
measures on occurrence of injuries among adolescents with multiple sclerosis. Design: pre-posttest
quasi-experimental research design was utilized in the current study. Setting: The study was
conducted in the multiple sclerosis unit (out patients clinics) at Al-Kasr Al-Ainy Hospital. Sample:
A purposive sample of 50 adolescents with MS aged from 12-18 years, free from any other central
nervous system disorders and had previous exposure to injuries (falling, bruises, sprains, cuts,
burns, choking, poisoning and drowning). Tools: Data collection was done through 1) Structured
Interviewing Questionnaire Sheet, 2) Pre/post Knowledge assessment Questionnaire, 3) Pre/post
Reported Practices Checklist and 4) Injures Occurrence Recording Sheet. Results: nearly half of
selected adolescents (46%) aged from 16-18 with mean= 15.1± 0.6. More than three quarters (76%)
of adolescents were female while 48% of them had the second rank in their families. Selected
adolescents with MS suffered from fatigue, numbness and tingling, pain, thinking, and learning
problems presented as 88%, 84%, 80%, and 80% respectively. 62% of adolescents with multiple
sclerosis were diagnosed less than 12 months. 92% of them were hospitalized and 36% of them
were due to complications. Exposed to falling, bruises, sprains, cuts, burns, choking, poisoning, and
drowning (46%, 60%, 44%, 34%, 38%, 12%, 6% 10% respectively) among the selected adolescents
with multiple sclerosis before intervention. Less exposed to injures as falling 24%, bruises 38%,
sprains 32%, cuts 22%, burns 22%, choking 6%, poisoning 6%, and drowning 4% respectively after
received nursing guidelines among the adolescence with MS. There were highly statistically
significant differences before and after nursing guidelines at P ≤ 0.01. Conclusion: The current
study concluded that the adolescents with MS who receive nursing guidelines for safety measures
had higher mean score of knowledge and reported practice than before with highly statistically
significant differences. Adolescents after receiving the nursing guidelines for safety measures less
exposed to injuries than before. Recommendation: Conducting nursing guidelines for safety
measures for adolescents with MS and their caregivers in MS unit (outpatient clinics / inpatient
section).and Applying educational program for adolescents with MS regarding developed nursing
guidelines for safety measures in different community setting as schools, family health centers.

Background: Medication error reporting is a critical healthcare issue globally. Although research
has established that a just culture is beneficial for raising safety in health care settings, it’s less clear
whether a hospital’s just culture can boost nurses’ willingness to report their own medication errors.
Aim: This study aims to explore the effect of the hospital’s just culture on nurses’ willingness to
report their own medication errors. Methods: A quantitative, cross-sectional, descriptive, and
correlational study design was adopted. A sample of 378 clinical nurses employed in Port Said,
Egypt, completed measures of the perceived just culture and willingness to report their own medication
errors. Data were analyzed using descriptive statistics, point-biserial correlations, and logistic
regression. Results: Nurses perceived their hospitals with a low level of just culture, and 55.8% of
them were unwilling to report their medication errors. Logistic regression revealed that perceived
hospital just culture among nurses increased significantly the odds of nurses’ willingness to report
their medication errors by 10.7%. Conclusion: Hospitals adopting the just culture boost their nurses’
willingness to report their medication errors. Recommendations: Hospital managers at all levels
should adopt just culture and implement all of its dimensions as a strategy to encourage nurses to
report their medication errors. In addition, nursing scholars should take the responsibility to
disseminate the just culture concept in hospitals through workshops, seminars, and educational
studies to increase awareness of the just culture concept.

Background: Pressure ulcers continue to be one of the most prevalent conditions in patients with orthopedic disorders, followed by immobilization. Aim: To evaluate the effect of pressure ulcer prevention through applying evidence-based nursing interventions for patients with orthopedic disorders. Setting: This study was implemented at the orthopedic department of Menoufia University Hospital, Egypt. Methods: A research design experiment was used in this study. A sample of 80 adult patients was randomly selected and split into two equal groups of 40 patients each. Three tools were utilized to collect data: tool (I): The Scio demographic and clinically structured interview questionnaire, including demographic data, personal hygiene, and a skin assessment observation check list (pressure ulcer signs and symptoms). (Tool 2): Predicting pressure sore risk using the Braden Scale (tool 3): The Numeric Pain Rating Scale. The results: The data analysis revealed that the intervention group had a statistically significant reduction in pain, redness, and edema of the skin in the study areas compared with the control group at the post-intervention period. As a result, the incidence of pressure ulcers among the study group was lower than in the control group. This means that the implementation of the evidence-based nursing interventions could prevent the occurrence of pressure ulcers in the intervention group. Conclusions: Implementation of an evidence-based nursing intervention could prevent the occurrence of pressure ulcers among the hospitalized patients in the orthopedic ward. Recommendations: Preventing pressure ulcers through evidence-based nursing interventions should be applied to an area of the skin in patients at risk for pressure ulcers.
